Laryngeal paralysis: Diagnosis, treatment and complications

Laryngeal paralysis is a common disorder seen primarily in older dogs of certain breeds. It has been shown that laryngeal paralysis is part of a more generalised neuropathy that also affects the oesophagus and long motor nerves. Diagnosis is based on clinical signs such as exercise intolerance, heat intolerance and more obvious respiratory obstruction with confirmation by direct visualisation of the larynx. Laryngeal assessment for paralysis can be difficult and somewhat subjective in nature but practical ways to perform a visual examination of the larynx are described. Patients with laryngeal paralysis present either with chronic signs or acutely collapsed. Each presentation is managed differently with stabilization of acutely collapsed patients an absolute requirement before surgical intervention is considered. The use of supplemental oxygen, cooling, sedation and even tracheostomy should be considered before definitive treatment is undertaken. Short-term medical management and surgical treatment options are discussed. Attendant complications include laryngeal oedema, laryngeal collapse, aspiration pneumonia and surgical failure. An awareness of potential complications and strategies to manage them when they occur substantially improve post-operative outcomes and reduce the stress associated with managing these cases.
